Altra Homecare of Kokomo is seeking a Personal Care Assistant in Wabash, IN to support patients at home with essential personal care services. You will assist with daily activities, monitor health, and maintain care documentation under supervision.

The role emphasizes compassion, safety, and clear communication. CPR certification and valid license are required by state guidelines; 1 year of experience is preferred. Flexible scheduling available.

Qualifications

• Must be 18 years of age or older.
• Valid Driver's License and Auto Insurance.
• CPR certification preferred or required per state guidelines.
• Read, follow written instructions and document care provided.
• State and federal training requirements to be complied with.

Responsibilities

• Assist with patient laundry when necessary.
• Provide medication reminders.
• Ensure patient safety and comfort at all times.
• Report changes in condition to supervisor promptly.
• Prepare light meals and assist with feeding as needed.
• Monitor and record patient temperature and vitals.
• Assist with ambulation and ROM exercises.
• Maintain documentation and follow care protocols.
• Follow privacy policies and infection control guidelines.
• Offer bathing assistance and light housekeeping.
• Provide companionship, including letter-writing and reading.
