Altra Homecare Group in Rensselaer, IN is seeking compassionate Personal Care Assistants to join our patient‑centered home health team. You will deliver essential personal care services, assist with medications reminders, feeding, mobility, and daily living activities while maintaining safety and dignity for our clients.

The ideal candidate is at least 18, holds a valid driver's license and auto insurance, has CPR certification, and prefers 1 year of relevant experience.

Qualifications

• Must be 18 years of age or older.
• Possess a valid Driver's License and Auto Insurance.
• Preferred: Minimum of 1 year of relevant work experience.
• Ability to read, follow written instructions, and document care provided.
• Current CPR certification.
• Comply with training requirements as per State and Federal laws.

Responsibilities

• Assist with patient laundry when necessary.
• Provide medication reminders.
• Ensure patient safety.
• Report significant changes in the patient's condition directly to their supervisor.
• Prepare light meals and assist with patient feeding when required.
• Monitor and record patient temperature.
• Assist patients with ambulation.
• Conduct range of motion exercises and other simple physical activities.
• Maintain comprehensive documentation and adhere to care protocols.
• Follow privacy policies and infection control guidelines.
• Offer bathing assistance.
• Perform light housekeeping.
• Provide companion services, including letter-writing and reading.
