Job summary

Mercy LIFE Norristown, part of Trinity Health PACE, is hiring a Personal Care Assistant to support seniors at home and in the community. You will provide compassionate personal care and health support to help participants remain independent and safe.

The role includes performing ADLs, coordinating with an interdisciplinary team, and assisting with mobility, meals, and transportation. Day shift, full-time, with every other weekend rotation.

Qualifications

• Minimum of one year of experience with frail or elderly population.
• Valid driver's license, current auto insurance, and reliable vehicle.
• Strong communication, organizational, and interpersonal skills.
• Ability to travel within the service area as needed.

Responsibilities

• Conduct ongoing participant assessments and monitoring
• Assist with activities of daily living, including: Toileting Grooming and personal hygiene Meal assistance
• Support participants with mobility, exercise, and physical activities
• Identify safety concerns and respond appropriately to emergencies
• Assist with recreational activities, meal service, and light housekeeping
• Communicate changes in participant condition to the interdisciplinary care team
• Escort participants to appointments and assist with transportation needs

What We’re Looking For

Required Qualifications:

• High School Diploma or GED
• Minimum of one (1) year of documented experience working with a frail or elderly population Candidates with less than one year of experience may be considered with appropriate training provided upon hire
• Valid driver's license, current auto insurance, and reliable personal vehicle
• Strong communication, organizational, and interpersonal skills
• Ability to work independently and adapt to changing needs
• Ability to travel within the service area as needed

Preferred Qualifications:

• Experience in long-term care, senior care, home care, or geriatric programs
