Job summary

CareOne at Sharon in Sharon, MA seeks a Certified Home Health Aide (CHHA) to support residents in daily living activities and comfort. The role includes ADL assistance, personal care, mobility transfers, meal support, and safe medication reminders, with travel as needed and hospice alignment.

The ideal candidate holds a State CHHA license, a high school diploma, and prior hospice/home health exposure is preferred. Competitive shifts offered with comprehensive benefits.

Responsibilities

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Assists with toileting; positioning and transfer activities; bed bound turning and positioning.
• Assists with prescribed exercises and ROM as taught by Hospice Nurse.
• Light housekeeping of personal unit; bed linen changes and patient laundry.
• Feeds and assists with meals; observes and reports patient condition changes.
• Assists with medications in compliance with regulations.

$17.00 to $21.01

Compensation will be based on, but not limited to, experience, qualifications, credentials, employment status, scheduled shift, and, if applicable, differentials related to a specific shift(s)/and or day(s)

Full Time, Part Time, Per Diem Shifts Available

The Certified Home Health Aide (CHHA) will be responsible for, but not limited to:

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Position Requirements

• High School Diploma or equivalent
• Must have a State Certified CHHA license
• Experience in hospice or home health, preferred
• No experience necessary but 1 year, preferred
• Must be willing to travel
• Must have a valid driver's license and reliable transportation
