Job summary

CareOne is seeking a Certified Home Health Aide (CHHA) in East Brunswick, NJ. This role supports residents with daily living activities, hygiene, mobility, nutrition and medication reminders under supervision.

The CHHA will document patient information and communicate with nursing staff, and must hold a valid CHHA license and be willing to travel. We offer full-time, part-time and per diem opportunities across shifts with a strong focus on compassionate, patient-centered care.

Qualifications

• High School Diploma or equivalent.
• State Certified CHHA license.
• Experience in hospice or home health preferred.
• 1 year experience preferred but not required.
• Willing to travel.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide or assist with ADL activities, personal hygiene and grooming.
• Assist with toileting, transfers and positioning; assist bed-bound residents.
• Support ROM and prescribed exercises as instructed.
• Light housekeeping of resident's room; change bed linens as needed.
• Assist with meals and feeding.
• Take and record vital signs under supervision.
• Report changes to Hospice Nurse and document appropriately.

Salary Range: $19-$25 per hour

CareOne at East Brunswick Assisted Living

The Certified Home Health Aide (CHHA) will be responsible for, but not limited to:

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Position Requirements

• High School Diploma or equivalent
• Must have a State Certified CHHA license
• Experience in hospice or home health, preferred
• No experience necessary but 1 year, preferred
• Must be willing to travel
• Must have a valid driver's license and reliable transportation
