Job summary

Ascend Hospice & Palliative Care NJ is hiring a Certified Home Health Aide (CHHA) for the North Team. The role includes ADL support, mobility assistance, routine nursing tasks, light housekeeping, meal support, and medication reminders within a hospice/home health setting.

Qualified candidates hold a State Certified CHHA license and are willing to travel. High school diploma or equivalent required; valid driver’s license and reliable transportation preferred. Equal opportunity employer.

• Experience in hospice or home health, preferred.
• No experience necessary but 1 year, preferred.
• Must be willing to travel.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Assists with toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with prescribed exercise and ROM activities as taught by the Hospice Nurse.
• Assists with light housekeeping of the individual's unit, including changing bed linen and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports changes to the Hospice Nurse and documents as required.
• Assists or reminds patient to take medication per regulations.

ThisCertified Home Health Aidewill be responsible for, but not limited to:

1. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.

7. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
8. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.
