Job summary

CareOne in Wayne, NJ is hiring a Certified Home Health Aide (CHHA) to support residents with ADL, hygiene, and personal care in a home-health setting. The CHHA will assist with toileting, transfers, ROM activities, medication reminders, and daily living tasks under Hospice Nurse supervision.

Requires a State Certified CHHA license and a high school diploma. Travel, driving license and reliable transportation are expected; hospice/home health experience is preferred but not required for all

Qualifications

• High School Diploma or equivalent.
• Must have a State Certified CHHA license.
• Experience in hospice or home health is preferred.
• No experience necessary but 1 year is preferred.
• Must be willing to travel and have a valid driver's license.

Responsibilities

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with prescribed exercise and ROM activities as taught by Hospice Nurse.
• Assists with light housekeeping and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports changes to Hospice Nurse and documents information on Home Health Aide Certification form.
• Assists or reminds patient to take medication in regulatory compliance.

Salary Range: $16.00-$25.00

Certified Home Health Aide (CHHA)

The Certified Home Health Aide (CHHA) will be responsible for, but not limited to:

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Position Requirements
