Job summary

Ascend Hospice & Palliative Care MA is hiring a Certified Home Health Aide (CHHA) to support residents in their homes, performing ADL assistance, personal care, and mobility support under Hospice Nurse supervision.

Must hold a State CHHA license; High School Diploma; willing to travel; valid driver’s license. Hospice or home health experience preferred; competitive salaries and benefits offered.

Qualifications

• Education: High School Diploma or equivalent.
• Hospice or home health experience preferred; travel willingness required.
• Valid driver's license and reliable transportation.

Responsibilities

• Provides or assists in ADL activities: personal hygiene, grooming, mouth, skin care and nails.
• Assists with toileting, positioning, transfers and turning of bed-bound residents.
• Assists with prescribed exercises and ROM as taught by Hospice Nurse.
• Light housekeeping of personal unit, including bed linen changes and patient laundry.
• Feeds and assists with meals.
• Measures temperature, pulse and respirations as supervised by Hospice Nurse.
• Reports changes in patient's condition to Hospice Nurse and documents accordingly.
• Assists or reminds patient to take medications in accordance with regulations.

Now Hiring - Certified Home Health Aide (CHHA)

This Certified Home Health Aide will be responsible for, but not limited to:

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education

High School Diploma or equivalent

Professional Experience

• Must have a State Certified CHHA license
• Experience in hospice or home health, preferred
• No experience necessary but 1 year, preferred
• Must be willing to travel
• Must have a valid driver's license and reliable transportation
