Job summary

United Counseling Service (UCS) seeks a Dedicated Specialized Services Provider to deliver individualized supports to a person with an intellectual disability or Autism in the community. The role involves 24/7 supervision for a high-needs individual across Battelle Crisis Stabilization House, an independent apartment, or hotel accommodations.

Under the Shared Living Program Coordinator, you will collaborate with clinical staff, encourage community participation, and ensure safety and compliance

Qualifications

• High School Diploma or Equivalent.
• Relevant experience in human services field preferred.
• LICENSE REQUIRED: Valid Driver's License

Responsibilities

• Provide respectful, professional one-to-one supports and supervise safety for individuals with intellectual disabilities or Autism Spectrum Disorder.
• Advocate for consumers, connect them to community resources, and support participation in community activities.
• Collect required data and complete daily/weekly time sheets and monthly documentation accurately.
• Collaborate with clinical staff to ensure mental health needs are addressed and treatment plans followed.
• Coordinate transportation and assist with personal care as needed while maintaining confidentiality and safety.
