Qualifications

• Bachelor's degree in social work, human services, or a related field (or equivalent experience).
• Must pass background checks including TB test and CORI.
• Basic computer skills using Microsoft Office, email, internet, and documentation entry.
• Strong interpersonal and communication skills to collaborate with participants and teams.
• Valid driver's license and reliable personal vehicle.

Responsibilities

• Conduct monthly home visits for program members and Care Providers, ensuring wellbeing and regulatory compliance.
• Complete all necessary documentation accurately and promptly to maintain records of each visit.
• Maintain open communication with assigned staff and coordinate with other service providers, such as day programs and work programs, to ensure comprehensive care.
• Make referrals to external resources when additional support is needed.
• Monitor the home setting according to regulatory guidelines to ensure safety and comfort for individuals with disabilities.
