Job summary

The Arc of Opportunity in Fitchburg, MA is seeking a bilingual, person-centered Plan Manager to help individuals achieve goals in the community. The role emphasizes advocacy, service coordination, and documentation in an electronic system.

The position is full-time with a 40-hour week, Monday–Friday 8:00 AM–4:00 PM; requires an Associate degree or equivalent, a valid driver's license, and proficiency with Microsoft Office and email communication.

Qualifications

• Associate degree or equivalent experience, preferably in a related field.
• Valid driver's license with an acceptable driving record and ability to pass background checks.
• Basic computer skills, including Microsoft Office, email communication, internet, and documentation entry.
• Ability to work independently and collaboratively as part of a team.
• Compassion, resourceful, and a strong desire to learn and grow.

Responsibilities

• The Person-Centered Plan Manager, also known as a case manager, works directly with the individual in the community to help them achieve their goals. Typically, approximately 70% of time is spent in the community.
• Connects the individual with the services they need to maintain and expand their independence. This may include community services, government-funded benefits, or other publicly available resources.
• Advocates for the individual ensuring that they receive the proper support for their situation.
• Completes administrative tasks on behalf of the individual, including consultations, documentation into an electronic record system, benefits eligible forms, and other related tasks.
