Job summary

The Arc of Opportunity in North Central Massachusetts is seeking a dedicated Person-Centered Plan Manager to support individuals with developmental disabilities in the community. You will partner with clients to identify goals, connect services, and advocate for necessary supports.

Typical schedule is Monday–Friday, 40 hours per week, with a focus on community-based work and documentation in electronic records. Strong communication, reliability, and compassion are essential.

Qualifications

• Associate degree or equivalent experience, preferably in a related field.
• Valid driver's license with clean driving record and ability to pass background checks.
• Basic computer skills, including Microsoft Office, email communication, internet, and documentation entry.
• Ability to work independently and collaboratively as part of a team.
• Compassion, resourceful, and a strong desire to learn and grow.

Responsibilities

• The Person-Centered Plan Manager works directly with the individual in the community to help them achieve their goals, spending about 70% of time in the community.
• Connects the individual with the services they need to maintain and expand their independence, including community services, government-funded benefits, or other resources.
• Advocates for the individual ensuring they receive the proper support for their situation.
• Completes administrative tasks on behalf of the individual, including consultations, documentation into an electronic record system, benefits eligible forms, and other related tasks.
