Job summary

Envision, Inc. seeks a Targeted Case Manager to deliver person-centered case management services for individuals with IDD in accordance with KDADS, HCBS, and related standards.

The TCM collaborates with individuals, families, guardians, providers, schools, and medical professionals to coordinate services promoting health, safety, independence, community inclusion, and informed choice. Strong communication and documentation skills are essential.

Qualifications

• Bachelor’s degree in a human services field preferred; equivalent experience will be considered.
• Minimum six months working with individuals with developmental disabilities.
• Knowledge of IDD services, HCBS, and person-centered planning preferred.

Responsibilities

• Develop and maintain Person-Centered Support Plans (PCSPs).
• Coordinate medical, behavioral health, educational, residential, employment, and community supports.
• Monitor services to ensure needs are met and supports align with PCSP.
• Advocate for the rights, dignity, preferences, and goals of the individual served.
• Assist individuals and families with accessing community resources, Medicaid, Social Security benefits, and waiver services.
• Maintain regular communication with individuals served, guardians, providers, and team members.
• Support transitions, referrals, discharges, and coordination of services.
• Complete accurate and timely documentation, billing, and case records.
• Report incidents and concerns according to agency and regulatory requirements.
