Qualifications

  • Bachelor's degree in human services, social work, psychology, healthcare administration, public health, education, business administration, or a related field preferred; equivalent experience may be considered.
  • 3–5 years of relevant experience in developmental disabilities services, human services, healthcare administration, program coordination, compliance, case management, or operations.
  • Experience supporting individuals with intellectual and developmental disabilities.
  • Working knowledge of the Maryland DDA service delivery system.
  • Experience with Person-Centered Plans, Service Implementation Plans, service authorizations, documentation requirements, and interdisciplinary team coordination.
  • Experience using electronic health record, case management, or documentation systems.
  • Strong ability to manage deadlines, records, calendars, documentation, and multiple priorities.

Responsibilities

  • Manage annual PCP calendar and coordinate meetings, annual PCP meetings, revisions, quarterly reviews, and interdisciplinary team meetings.
  • Review PCPs for approved outcomes, goals, services, staffing supports, health and safety needs, behavioral supports, and documentation requirements.
  • Develop, update, and maintain SIPs aligned with PCPs and service authorizations.
  • Translate PCP outcomes and goals into measurable implementation strategies for DSPs and program staff.

InspiredOptions Inc. is seeking an experienced, organized, and detail-oriented Person-CenteredPlan (PCP) Coordinator to support the delivery of high-quality,person-centered services to individuals with intellectual and developmentaldisabilities.

Thisposition is ideal for a human services professional with strong knowledgeof Maryland Developmental Disabilities Administration (DDA) services,Person-Centered Plans, Service Implementation Plans (SIPs), serviceauthorizations, documentation, and compliance. The PCP Coordinator will serve as a centralpoint of coordination among program participants, Coordinators of CommunityServices (CCSs), families and guardians, program leadership, nursing staff,Direct Support Professionals (DSPs), billing and quality assurance teams, andother stakeholders.

As thePCP Coordinator, you will manage and monitor the full PCP process and helpensure each participant's approved services are translated into measurable,person-centered supports.

Keyresponsibilities include:

  • Manage theannual PCP calendar and coordinate preparation meetings,annual PCP meetings, revisions, quarterly reviews, and interdisciplinaryteam meetings.
  • Review PCPs forapproved outcomes, goals, services, staffing supports, health and safetyneeds, behavioral supports, community participation, and documentationrequirements.
  • Develop,update, and maintain Service Implementation Plans (SIPs) thatalign with approved PCPs and service authorizations.
  • Translate PCPoutcomes and goals into clear, measurable implementation strategies forDSPs and program staff.
  • Develop andmaintain systems for tracking participant outcomes, goals, progress, andrequired documentation.
  • Review servicenotes and program documentation to identify gaps in goal implementation,service delivery, and outcome progress.
  • Review andmonitor service authorizations, approved hours, units, effective dates,expiration dates, and service changes within LTSSMaryland.
  • Collaboratewith appropriate staff to maintain accurate service and authorizationinformation within CIMS.
  • Develop andcoordinate Detailed Service Authorization Tool (DSAT) informationbased on participant needs and approved/requested services.
  • Work withnursing staff to ensure health, safety, HRST, medication, and delegatednursing considerations are appropriately incorporated into serviceplanning.
  • Support newparticipant admissions by reviewing PCPs, authorizations, DSATinformation, health and safety needs, and service requirements.
  • Train DSPs,House Managers, Program Coordinators, and other staff on PCP outcomes, SIPimplementation, documentation standards, and service expectations.
  • Conduct routinequality assurance reviews and audits of PCPs, SIPs, service notes,authorizations, outcome documentation, and related records.
  • Identifydocumentation or authorization discrepancies and coordinate timelyresolution with program leadership, CCSs, nursing, billing, and otherstakeholders.
  • Maintainorganized, current, confidential, and audit-ready participant records.
  • Supportdocumentation and authorization readiness for billing and regulatoryreviews.
  • Assist with PCPand SIP updates following hospitalizations, significant incidents,behavioral changes, health changes, or other changes in participant needs.

Required Qualifications

  • Bachelor'sdegree in human services, social work, psychology, healthcareadministration, public health, education, business administration, or arelated field preferred. Equivalent relevant experience may beconsidered.
  • Minimumof 3–5 years of relevant experience in developmentaldisabilities services, human services, healthcare administration, programcoordination, compliance, case management, or operations.
  • Experiencesupporting individuals with intellectual and developmentaldisabilities.
  • Workingknowledge of the Maryland DDA service delivery system.
  • Experiencewith Person-Centered Plans, Service Implementation Plans, serviceauthorizations, documentation requirements, and interdisciplinary teamcoordination.
  • Experienceusing electronic health record, case management, or documentation systems.
  • Strong abilityto manage deadlines, records, calendars, documentation, and multiplepriorities.

Strongly Preferred

  • Directexperience working with a Maryland DDA provider.
  • Familiaritywith LTSS Maryland, CIMS, DSAT, HRST, and DDA billing-relatedprocesses.
  • Experiencedeveloping or auditing Service Implementation Plans.
  • Experience withquality assurance, regulatory compliance, documentation audits, or billingreadiness.
  • Experiencetraining DSPs or other human services professionals.
  • Experience withEHR workflows, documentation templates, and data-tracking systems.
  • Master's degreein a related human services, healthcare, education, or administrativediscipline.

  • Ability to maintain confidentiality and professional boundaries.
  • Ability to workindependently while effectively collaborating within an interdisciplinaryteam.
