Job summary

Amivie, LLC is seeking a Designated Coordinator to execute a person-centered approach, ensuring each plan meets the individual’s needs, preferences, and goals.

You will facilitate intakes, develop and implement CSSP, SMA, and IAPP, keep licensing signatures, maintain files, and coordinate with the team to protect client well-being. This role requires a degree, license, and English fluency, with supervisory experience preferred.

Qualifications

• Associate’s degree is required.
• Fluent in English and able to pass a background check.
• Must meet 245-D licensing designation requirements.
• Valid driver's license and reliable access to a vehicle.
• Minimum one year supervisory experience preferred.
• Experience with mental health and developmental disabilities is preferred.

Responsibilities

• Facilitate and/or complete client intakes with a person-centered served plan.
• Develop, write, and implement each client's person-centered support plans (CSSP, SMA, IAPP).
• Ensure signatures comply with 245-D licensing rules.
• Maintain client files and prepare quarterly/annual reports.
• Provide staff orientation, training, and supervision.
• Monitor target symptoms and coordinate psychiatric appointments.
• Be available for consultation during/after hours.
• Perform physical tasks as required (lifting, transfers).
