Qualifications

• Two years of experience in social work, nursing, health sciences, health education, or related field.
• Paraprofessional with more than 7 years of directly related progressive work experience.
• Bachelor’s Degree preferred with a recognized major and minimum GPA 2.5.

Responsibilities

• Coordinate panels of patients with special service needs and implement the Health Action Plan (HAP).
• Assist patients in navigating health, behavioral health, social services systems, housing and transportation.
• Provide health promotion services and education for patient self-management per HAP.
• Assist in housing navigation, tenancy sustaining services and coordinating transitions.
• Support the Health Home Program initiatives and data reporting.
