Job summary

Qualifications

Responsibilities

• Monitor staff and consultant caseloads; assign SWAN referrals.
• Review SWAN documents for quality; submit to county agency.
• Provide support to staff and consultants for service completion.
• Lead and problem-solve issues beyond individual scope.
• Ensure timely SWAN benchmarks and billing.
• Attend mandatory SWAN and Delta meetings and trainings.
• Maintain compliance with Federal and State laws; coordinate with DHS and agencies.
