Heartshare is seeking a dedicated Case Planner to help children achieve permanency and safety in care. You will coordinate services, liaise with ACS and community resources, and partner with families and clinicians.

You will work with medical, mental health, and education teams, document progress, attend court hearings, and support reunification efforts through collaborative case management. Applicants should be able to travel across New York City by public transit and occasionally evenings.

Qualifications

• Bachelor's degree required.
• Knowledge of Child Welfare System preferred.
• Excellent written, oral, organizational, and interpersonal communication skills.
• Proficiency in Microsoft Office, including Outlook and Excel.
• Computer proficient; knowledge of Connections.

Responsabilités

• Creates service plans addressing safety, behavioral, educational and permanency needs with child, family and foster family.
• Monitors safety and progress; documents in progress notes and case files.
• Schedules and supervises mandated visits; conducts interviews with child and families; performs field visits as needed.
• Attends weekly supervision and follows supervisor directions.
• Attends Family Court hearings and provides testimony; supports court directives.
• Participates in Family Team Conferences and collaborates with Integrated Services to ensure medical/mental health needs are monitored.
• Uses agency and external data to measure performance and outcomes.
