Job summary

Jobtailor is seeking an RN to provide direct clinical care to peritoneal dialysis patients across assigned partner clinics in the Northeast region. You will educate patients and caregivers on PD procedures and self-care, and manage a PD caseload with careful assessments and care planning.

The role includes home visits, on-call support, and collaboration with interdisciplinary teams to ensure high-quality outcomes, with a flexible schedule for regional coverage.

Qualifications

• RN with active license in the practice state.
• Dialysis experience preferred (PD, HHD, chronic or acute).
• Background in patient education or teaching preferred.
• Experience in case management or telehealth preferred.

Responsibilities

• Rotate across assigned partner clinics in the Northeast region.
• Provide direct clinical care to peritoneal dialysis patients.

Requirements

• Valid insurance

• Patient education or teaching background preferred
• Case management or telehealth experience preferred
• CNN or CDN certification preferred
