Job summary

Jobtailor is seeking a Registered Nurse with nephrology/home dialysis experience to empower and educate patients on home dialysis modalities. Qualifications

  • Graduate of an accredited School of Nursing.
  • Current state RN licensure.
  • CPR BLS Certification.
  • Must meet the practice requirements in the employed state.
  • Entry level for RNs with less than 2 years of Nephrology Nursing experience.
  • Minimum of 12 months RN experience for assigned oversight duties.
  • Home dialysis therapy experience preferred.
  • Willingness to participate in on-call rotation including nights/weekends/holidays.
  • Ability to lift and move heavy equipment and patients.

Responsibilities

  • Empower, engage, and train dialysis patients on home dialysis therapies.

  • Adjust treatment plans and notify supervisors or physicians as needed.
  • Educate patients and families about transplant and treatment modality options.
  • Rotate coverage with licensed home therapies staff and participate in on-call coverage.
  • Perform other duties assigned by the Supervisor.

Job description

  • Empower, engage, and train dialysis patients on peritoneal and home hemodialysis using state-of-the-art training programs and equipment
  • Provide and coordinate clinically competent care, including assessment, planning, intervention, and evaluation for assigned patients
  • Assess and manage patient responses to home dialysis training and treatment according to prescribed protocols
  • Communicate patient-related issues to physicians as needed
  • Participate in decision‑making, teaching, leadership, quality improvement, staff training, and new‑staff orientation
  • Collect, analyze, and document dialysis data in patient medical records
  • Adjust or modify treatment plans and notify supervisors or physicians as needed
  • Assess and provide ongoing patient and family education
  • Deliver safe and effective dialysis therapy in collaboration with patients, home care partners, and care staff
  • Administer prescribed medications and document justification and effectiveness
  • Initiate or assist with emergency response measures
  • Ensure correct laboratory collection, processing, and shipping procedures
  • Update nursing assessments and plans of care with the Interdisciplinary Team
  • Educate patients about transplant and treatment modality options
  • Identify, evaluate, select, and educate Home Dialysis candidates and Home Partners
  • Assess home‑environment barriers and partner/family readiness for home dialysis
  • Train patients and Home Partners in operation and maintenance of home dialysis equipment and supplies
  • Rotate coverage with licensed home therapies staff and participate in on‑call coverage
  • Perform other duties assigned by the Supervisor
Requirements
  • Graduate of an accredited School of Nursing
  • Current appropriate state licensure
  • Current or successful completion of CPR BLS Certification
  • Must meet the practice requirements in the state in which he or she is employed
  • Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse
  • Minimum of 1-year experience as a Registered Nurse (preferred)
  • Home dialysis therapy experience (preferred)
  • Must have a minimum of 12 months experience as a RN for assigned oversight duties
  • Successful completion of 3 months experience as a RN in home peritoneal dialysis and / or hemodialysis for assigned oversight duties
  • Ability to provide direct patient care involving heavy lifting and moving equipment and patients
  • Ability to participate in on‑call rotation, night, weekend, and holiday coverage
  • Ability to travel to training sites, other facilities, or patient homes as required
