Job summary

The Pennsylvania State University in Hershey, PA is seeking a part-time Care Coordinator for the Perinatal TiPS Program at the Northeast Drive Clinic. You will facilitate outreach, coordination, and liaison activities between community providers, families, and internal teams to ensure seamless service delivery.

Primary duties include coordinating calls and meetings, onboarding staff, maintaining records, and supporting program outreach to improve perinatal mental health services.

Qualifications

• Strong organizational, communication, and interpersonal skills.
• Proficiency in Microsoft Office Suite (Excel, Publisher, Access) and other software tools (Beacon database, Qgenda).
• Ability to handle confidential information with professionalism.
• Experience in behavioral health, healthcare coordination, or a related field is preferred.

Responsibilities

• Field incoming provider calls, route to appropriate on-call physicians, and document calls in databases (Beacon, ACCESS, Excel).
• Call families to coordinate services, provide provider lists, and update PCP offices on patient statuses.
• Fax or mail relevant patient and provider information as needed.
• Plan and coordinate outreach activities, networking events, and maintain professional relationships with behavioral health providers, insurance companies, and community organizations.
• Maintain up-to-date resources, including a provider resource list and newsletters, and represent TiPS at statewide and local meetings.
• Coordinate physician on-call schedules and manage notifications using scheduling tools and communication systems.
• Train new staff, organize team onboarding materials, and ensure program resources are current.
• Maintain accurate records of program expenditures and assist with the development of marketing and outreach materials.
• Organize and facilitate monthly staff meetings, care coordinator subgroup meetings, and statewide TiPS program meetings.
• Take meeting minutes, prepare agendas, and provide updates on outreach and program outcomes.
• Review and analyze program data to support strategic outreach efforts.
• Schedule evaluations and therapy intakes, send rating scales to families, and ensure physicians have necessary documentation.
• Coordinate interpreters and provide administrative support for patient-related needs.
