Job summary

Covenant HealthCare is seeking a Physical Therapy Assistant in Saginaw, Michigan. In this role, you will provide therapy treatments according to the Physical Therapist’s plan of care, ensuring excellent customer service.

Job description

The role of the Physical Therapy Assistant is to provide therapy treatments as established by the Physical Therapist’s plan of care and to teach the patient and/or family how to carry out home programs prescribed. The Physical Therapy Assistant demonstrates excellent customer service performance in that his/her attitude and actions are at all times consistent with the standards contained in the Vision, Mission and Values of Covenant and the commitment to Extraordinary Care for Every Generation.

Responsibilities
  • Plan, delegate and supervise physical therapy activities under the direction of the physical therapist; discuss established discharge goals during treatment and provide information on goals met or unmet to the physical therapist.
  • Participate in educating patients and family/caregivers per the prescribed physician’s plan of care; instruct patients in the care and use of equipment as directed by the physician and physical therapist.
  • Display a high regard for personal dignity, diversity and uniqueness of individuals, including cultural/religious sensitivity, confidentiality, privacy, and age‑specific competencies.
  • Evaluate patient needs continuously, recognize changes that require adjustments to the plan of care, and notify the Physical Therapist of these needs; make joint visits with the Physical Therapist as needed.
  • Maintain interdisciplinary communication with other disciplines via written and verbal means.
  • Complete team‑meeting reports, including activities, patient responses to treatment and progress toward goals.
  • Coordinate scheduling of patients.
  • Complete required agency forms to record time and patient progress in a timely manner; assist with other clinical record‑keeping as deemed appropriate.
  • Participate in in‑service programs and staff meetings as required.
  • Maintain confidentiality with all information in an appropriate manner.
  • Contribute to organization success targets for patient satisfaction and net operating margin by providing excellent customer service performance.
Qualifications
Education / Experience
  • Graduate of a two‑year college‑level program approved by the American Physical Therapy Association.
  • At least one year of experience as a Physical Therapist Assistant.
Previous Homecare Experience Preferred
  • BLS certified or eligible.
Knowledge, Skills & Abilities
  • Homecare knowledge preferred.
  • Ability to type 25 WPM.
  • Demonstrate critical thinking skills.
  • Comfortable with basic computer programs and e‑mail.
  • Strong interpersonal skills.
  • Good organizational skills.
  • Self‑motivated.
Working Conditions / Physical Demands
  • Ability to maintain punctual attendance consistent with ADA, FMLA, and other federal, state, and local standards.
  • Constant lifting 0‑10 lbs.
  • Constant standing, walking, hearing, near vision, mid‑range vision, far vision, visual accommodation, color vision, and field of vision.
