Job summary

ABL Health Care is seeking a Part Time (Per Diem) Speech Therapist to join our Home Health team in Vallejo/ Berkeley area. You will assess speech and communication needs, create treatment plans, and support patient progress in the field and remotely.

The role requires an active CA OT license (note: if this is a direct typo, please align to Speech Therapist requirements in CA) and travel across the service area. Per diem schedule with competitive benefits and a supportive back office.

Qualifications

• Current OT License to practice in California - Required
• Experience in Home Health or Hospice care - 1 Year Required
• Current CA Driver’s License
• Able to travel

Responsibilities

• Provide, coordinate, and oversee skilled speech therapy services as per the patient’s individualized plan of care, in collaboration with the Interdisciplinary Team.
• Conduct initial and ongoing evaluations of patient status.
• Develop and implement treatment plans, monitor patient progress, and report any changes in condition to the Physician, Case Manager, and/or Director of Nursing.
• Deliver educational information to patients and their families regarding speech and language disorders and treatment plans.
• Maintain accurate clinical records, submitting documentation in a timely manner per company policy.
• Adhere to all company policies and procedures to ensure the highest standards of care.
