ABL Health Care in San Rafael, CA is seeking a Speech Therapist (ST) for home health per diem part-time. You will assess speech and communication needs, develop individualized treatment plans, and work with the Clinical Director and team in the field and remotely.

The role offers flexible per diem scheduling, travel within a 25-mile radius, and comprehensive benefits including medical, dental, vision, 401(k), life insurance, and more.

Qualifications

• Current OT License to practice in California – Required.
• Experience in Home Healthcare – 1 Year Required.
• Current CA Driver’s License.
• Able to travel within a 25 mile radius on a regular basis.

Responsibilities

• Provide skilled speech therapy services as per the patient’s plan of care, in collaboration with the Interdisciplinary Team.
• Conduct initial and ongoing evaluations of patient status.
• Develop and implement treatment plans, monitor patient progress, and report changes to the Physician, Case Manager, and Director of Nursing.
• Deliver educational information to patients and families regarding speech and language disorders and treatment plans.
• Maintain accurate clinical records, submitting documentation in a timely manner per company policy.
• Adhere to all company policies and procedures to ensure the highest standards of care.
