ABL Health Care is seeking a Part Time Speech Therapist (ST) for Home Health in the Vallejo/Bay Area region. You will assess patients’ speech and communication needs, create individualized treatment plans, and support progress toward improved quality of life in the field and remotely.

You’ll work with the Clinical Director and nursing staff, travel within the region, and use company devices and back-office support to deliver patient-focused care.

Responsibilities

• Provide, coordinate, and oversee speech therapy services as per patient care plan with the interdisciplinary team.
• Perform initial and ongoing evaluations of patient status.
• Develop and implement treatment plans, monitor progress, and report changes to physicians and care managers.
• Deliver educational information to patients and families about speech and language disorders and treatment plans.
• Maintain accurate clinical records and submit documentation per company policy.
• Adhere to all company policies and procedures to ensure high standards of care.

Position Overview

We are seeking an experienced, self-driven Part Time (Per Diem) Speech Therapist (ST) to join our Home Health team and provide exceptional care throughout the region. As a ST, you will assess patients’ speech and communication needs, develop individualized treatment plans, and support their progress to enhance their quality of life. You’ll collaborate closely with our Clinical Director and Clinical Staff, working independently in the field and remotely.

Responsibilities

• Provide, coordinate, and oversee skilled speech therapy services as per the patient’s individualized plan of care, in collaboration with the Interdisciplinary Team.
• Conduct initial and ongoing evaluations of patient status.
• Develop and implement treatment plans, monitor patient progress, and report any changes in condition to the Physician, Case Manager, and/or Director of Nursing.
• Deliver educational information to patients and their families regarding speech and language disorders and treatment plans.
• Maintain accurate clinical records, submitting documentation in a timely manner per company policy.
• Adhere to all company policies and procedures to ensure the highest standards of care.

Requirements

• Current OT License to practice in California - Required
• Experience in Home Health or Hospice care - 1 Year Required
• Current CA Driver’s License
• Able to travel
