Job summary

Mount Sinai West in New York is seeking a Per Diem Social Worker for Saturdays (9am-5pm) to assist patients and families in understanding illness and resolving personal and environmental barriers to care.

The role involves collaboration with care teams, discharge planning, community referrals, and educational activities within the hospital setting. LMSW/LCSW licensure in NY may be required.

Qualifications

• Master's degree in social work from an accredited institution.
• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in New York State. Some positions may require the LCSW.

Responsibilities

• Interview patients and families to assess social, psychological and environmental situations.
• Provide social work services including individual and/or group treatment sessions.
• Direct patients/families to community resources for financial aid, counseling and care.
• Conduct discharge planning and arrange placement in appropriate facilities.
