The Mount Sinai Hospital is seeking a Social Worker for Inpatient Med/Surg units in New York. You will deliver psychosocial services with a focus on complex discharge planning, collaborating with an interprofessional team to support patients and families facing acute and chronic illness.

Responsibilities include contributing to hospital initiatives, program development, and coverage across medicine/surgery units. NYS LMSW/LCSW licensure is required, with inpatient experience preferred.

Qualifications

• Licensed as NYS LMSW or LCSW.
• Excellent clinical, collaborative, and organizational skills.
• Experience providing psychosocial services to patients with acute or chronic illnesses.
• Ability to develop and implement safe, timely discharge plans in a fast-paced environment.
• Ability to coordinate multiple disciplines for complex family meetings.
• Knowledge of crisis, family, and systems theories.
• Inpatient hospital experience preferred.
• Internal candidates: minimum one year in current position unless Float in Medicine.
