Job summary

The position is full-time, day shifts (Thursday–Saturday 7:30am–8:30pm). NY LMSW license and MSW degree are required, with experience in medical settings preferred.

Qualifications

• MSW from an accredited school of social work.
• Licensed LMSW per NYSED.
• Experience in medical/health care setting preferred (1 year).
• Knowledge of social work services, theories, and community resources.

Responsibilities

• Assess and evaluate patients/families' social and psychosocial needs.
• Develop options and treatment plans promptly.
• Provide direct service and casework to patients and families.
• Identify and refer to community resources.
• Monitor high-risk patients and coordinate multiple services.
• Advocate for patients within the care team and community.
• Participate in rounds, conferences, and meetings.
• Document in medical records per standards.
• May supervise social work interns.
• Knowledge of child protection, elder abuse, domestic violence policies.
