Job summary

Qualifications

• Master's degree in social work from an accredited institution.
• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in New York State.
• Some positions may require the LCSW.

Responsibilities

• Interview patients and families to assess social, psychological and environmental needs.
• Provide social work services and conduct individual or group sessions.
• Direct patients to community resources for financial assistance or care.
• Conduct discharge planning and arrange placement in appropriate facilities.
• Collaborate with health care teams and participate in conferences and committees.
• Develop quality improvement initiatives with staff.
