Job summary

Masonicare at Wesley Heights in Shelton, CT seeks a RN for per diem shifts to provide and coordinate care focused on residents’ optimal health in an assisted living setting.

The role requires independent practice, strong communication, and the ability to educate residents and families. Responsibilities include assessments, care planning, orders, and supervision of care staff, with on-call duties as needed.

Qualifications

• Associates Degree in nursing; BSN preferred.
• Current RN license in CT; CPR certification.
• Sufficient nursing experience in a clinical setting.

Responsibilities

• Perform initial and ongoing nursing assessments to establish resident care plans.
• Develop, implement and evaluate resident plans of services.
• Transcribe, implement and evaluate physician orders promptly.
• Educate and supervise residents and families regarding AL procedures.
• Coordinate the plan of services and maintain continuity of care.
