Job summary

Masonicare at Wesley Heights in Shelton, CT is seeking a licensed practical nurse for per diem shifts. Under supervision of the ALSA Director, you will provide nursing care, support resident services plans, and collaborate with the care team to ensure quality resident outcomes.

Responsibilities include data collection for care planning, medication oversight with RN coordination, education for residents and families, accurate charting, and supervision of assistant staff when needed.

Qualifications

• Graduate of accredited School of Nursing
• 1-2 years clinical experience preferred
• Current LPN license; CPR certification

Responsibilities

• Contributes to resident care plans by collecting data and participating in care planning meetings.
• Communicates with residents, families and health care team to meet needs.
• Provides education, supervision and counseling to residents and families regarding procedures and care needs.
• Documents accurate information in the client service record.
• Conducts supervisory visits of resident care assistants and related staff.
• Participates in education, inservices, meetings and workshops to enhance skills.
• Oversees medication management in collaboration with ALSA Director/RN and others.
• Performs other duties as assigned.
