Job summary

Healthcare Nursing Center is seeking an MDS Coordinator (RN) to ensure accurate and timely completion of MDS and related assessments. The role supports an interdisciplinary team and drives improvement goals through meticulous chart reviews, meetings, and coordination of RAI processes.

Responsibilities include managing calendars, conducting assessments, developing care plans, and participating in Medicare and PDPM-related activities. CPR-certified clinicians with MA licensure are preferred.

Qualifications

  • RN licensed in the State of Massachusetts.
  • Minimum of three years of geriatric nursing or rehabilitation experience preferred.
  • CPR certification.
  • Ability to read, write, and comprehend English; follow oral and written instructions.
  • Fluent knowledge of MS Office suite and basic office equipment.

Responsibilities

  • Maintain care conferences calendar and conduct interdisciplinary meetings as needed.
  • Conduct comprehensive assessments of residents through medical record review, interviews, and resident evaluation.
  • Complete MDS with MDS-RAPS and resident summaries; develop resident care plans.
  • Ensure timely completion of all RAI processes and transmission per regulations.
  • Lead case management for Managed Care, as applicable.
  • Participate in CMI and billing meetings.
  • Communicate PDPM recommendations as appropriate.
  • Review final validation reports from the State and make corrections as needed.
  • Participate in weekly Medicare Meetings and communicate pertinent MDS information.
  • Review each Medicare resident for appropriateness of Medicare coverage.
  • Obtain monthly Quality Indicator Reports and provide to nursing management.

MDS Coordinator (RN) - Part-time and Full-time Opportunities Available

As a MDS Coordinator (RN) you will play a critical role in ensuring complete, accurate, and timely completion of clinical assessments. As a key member of our interdisciplinary health care team, you will support both short and long-term improvement goals.

Responsibilities:
  • Maintain care conferences calendar and conduct interdisciplinary meetings as needed

  • Conduct comprehensive assessments of residents through medical record review, interviews, and resident evaluation

  • Complete Minimum Data Set (MDS) with appropriate Resident Assessment Protocols (RAPS) and resident summaries, and develop resident care plans

  • Ensure timely completion of all aspects of the RAI process and timely transmission per State and Federal regulations

  • Lead case management for Managed Care, as applicable

  • Participate in CMI and billing meetings

  • Communicate PDPM recommendations as appropriate

  • Review final validation reports from the State and make corrections as needed

  • Participate in weekly Medicare Meetings and communicate pertinent MDS information

  • Review each Medicare resident for appropriateness of Medicare coverage

  • Obtain monthly Quality Indicator Reports and provide them to nursing management

  • Perform other duties as directed by management

Requirements:
  • RN licensed in the State of Massachusetts

  • Minimum of three (3) years of education or experience in geriatric nursing or rehabilitation preferred

  • CPR certified

  • Ability to read, write, and comprehend English; ability to follow oral and written instructions

  • Fluent knowledge of the MS Office suite and other office equipment

  • Ability to work hours as scheduled based on the requirements of the position assignment
