Responsibilities

• Assess, plan, implement and evaluate patient care needs. Includes triaging calls, patient education, taking histories, administering meds, wound care and vital signs.
• Document care in health record per standards.
• Maintain professional development and participate in policy/procedures.
• Contribute to quality improvement, safety and infection prevention programs.
• Provide appropriate responses to urgent or emergency situations.
• May screen patient inquiries regarding care, prescriptions, and referrals.
