Job summary

Live Well Home Care is actively seeking a Per Diem/PRN Registered Nurse (RN) to coordinate Start of Care, discharge planning, and resumption of care. This in-person role involves direct patient care, medications, diagnostics, and vital sign monitoring.

Ideal candidates hold an active RN license with at least 2 years of home care experience, strong communication skills, and a clean background/driving record. Mileage is reimbursed and scheduling is flexible to meet patient needs.

Responsibilities

• Coordinate Start of Care, Re-certification, Discharge, and Resumption of care.
• Provide direct patient care, including medications, tests, and vital signs.
• Collaborate with physicians and healthcare professionals to implement care plans.
• Educate patients and families about conditions and preventive care.
• Assist with discharge planning and post-discharge care coordination.
• Provide specialized care for dementia or cognitive impairments.
