Job summary

Day Kimball Health is hiring a Per Diem Registered Nurse, Case Manager in Putnam, CT. The RN Case Manager will work within the Case Management Department to assess patient needs, develop and implement care plans, and optimize post-discharge outcomes across the continuum of care.

The role focuses on coordinating resources, negotiating services, and performing utilization reviews to ensure appropriate level of care. Minimum three years nursing experience required; CCM preferred.

Qualifications

• Experience in identifying and addressing case management needs with patients and families.
• Ability to develop and implement comprehensive care plans and evaluate post-discharge needs.
• Strong knowledge of utilization reviews and documentation requirements.

Responsibilities

• Assess patients’ continuum-of-care needs and determine home care requirements.
• Plan and coordinate care with the healthcare team and external partners.
• Monitor outcomes and evaluate the effectiveness of the care plan during an episode of care.
• Assist with transfers to acute care and rehabilitation facilities and coordinate outpatient services.
