Job summary

Constellation Home Care is seeking a compassionate Registered Nurse (RN) Case Manager for field-based work in Stamford, CT. You will drive to patients' homes, assisted living facilities, and skilled nursing facilities, performing head-to-toe assessments and coordinating plans of care.

The role requires Oasis familiarity, strong clinical skills, and ability to manage admissions, re-certifications, discharges, and education for patients and caregivers.

Qualifications

• Registered Nurse licensed in the state of employment.
• 1–2 years of RN experience; home care preferred.
• OASIS experience strongly desired.
• Experience managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care a plus.
• Have reliable transportation and be willing to commute to provide in-home patient care.
• Some weekend days are required, and summer and winter holiday commitments as needed.
• Basic knowledge of iOS operating system.

Responsibilities

• Provide skilled nursing and head-to-toe assessments to adults and geriatric patients.
• Oversee medication administration and wound care as prescribed.
• Visit homes for admissions, re-certifications, and discharges for assigned caseload.
• Develop the Plan of Care with other health care professionals.
• Use current OASIS assessment for admissions, re-certifications, transfers, and discharges.
• Educate patients and caregivers on disease processes, plan of care and treatments.
• Report to Clinical Manager daily.
• Document using the provided iPad within required time frame.
