Job summary

Day Kimball Health is seeking a Per Diem Registered Nurse, Case Manager to join our Case Management Department in Putnam, CT. You will work with a collaborative team to assess needs, develop care plans, and coordinate services across the continuum of care.

Responsibilities include negotiations, monitoring, and facilitating post-discharge needs while optimizing outcomes and cost-effective care. Per diem requires a minimum monthly hours commitment and a day shift.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Minimum of three years nursing experience in hospital or homecare.
• Current AHA CPR or equivalent within 3 months of hire.
• CCM certification preferred.

Responsibilities

• Assess patients’ continuum-of-care needs by chart reviews and interviews.
• Plan and implement care plans in collaboration with the healthcare team.
• Coordinate resources and referrals to meet identified goals.
• Monitor outcomes and utilization; evaluate readmission risks.
• Assist transfers to acute care and rehabilitation facilities.
