Job summary

Day Kimball Health is looking for a Per Diem Registered Nurse, Case Manager to join our Case Management Department in Putnam, CT. This role includes assessing patient needs and managing care plans within a collaborative team.

The RN Case Manager will coordinate services and monitor care to optimize patient outcomes. A minimum of three years of nursing experience is required. Benefits include sick time, pet insurance, and a 401K plan.

Qualifications

• Minimum of three years nursing experience in either a hospital or homecare setting required.
• Membership and participation in professional organizations preferred.
• Current AHA Healthcare Provider CPR certification or equivalent within 3 months of hire required.

Responsibilities

• Assess patients’ continuum-of-care needs by conducting comprehensive chart reviews.
• Plans continuum of care in collaboration with the healthcare team.
• Implements care plans and coordinates resources to achieve identified goals.
• Monitors and evaluates the effectiveness of the continuum of care plan.
• Performs utilization management functions such as reviewing admissions.
