Job summary

Hackensack Meridian Health is seeking a RN Case Manager, Per Diem in Hackensack, NJ to coordinate patient care activities and ensure compliance with evidence-based standards and regulatory requirements. Qualifications

• BSN preferred; 1 year experience for non-BSN HackensackUMC employees.
• Current New Jersey Registered Nurse license required.
• BLS certification required.
• Minimum 2 years in Case Management in an acute care setting preferred.
• Strong collaboration and communication within a multidisciplinary team.

Responsibilities

• Coordinate patient care progression throughout the continuum.
• Facilitate collaborative management of patient care and remove barriers.
• Apply process improvement methodologies to evaluate care outcomes.
• Communicate with physicians, nursing, and ancillary staff.
• Evaluate care against Length of Stay metrics.
• Collaborate with Admitting for accurate patient demographic and insurance information.
• Communicate the medical plan of care and discharge plan to patients and families.
• Assess and plan for discharge needs and arrangements.
• Complete utilization management for assigned patients.
• Monitor length of stay and clinical resource use.
