Job summary

Hackensack Meridian Health is seeking an RN Case Manager PD to coordinate patient care across the continuum, aligning with evidence-based standards and regulatory requirements. You will integrate utilization management, care coordination, and discharge planning across units such as ETD, Observation, PACU, or others as directed.

Responsibilities include communicating with physicians, nursing and ancillary staff, leading discharge planning conferences, and ensuring accurate patient demographics

Qualifications

• Case Manager, Registered Nurse; NLN/AACN education is a plus.
• BSN preferred or 1 year experience for non-BSN HackensackUMC employees.
• Excellent interpersonal communication and negotiation skills.
• Strong analytical, data management and PC skills.
• Knowledge of discharge planning, utilization management and managed care.
• Understanding of pre- and post-acute care venues.
• Strong organizational and time management skills.
• Ability to work independently and exercise sound judgment.
• Weekend rotation to meet department needs; evenings as required.
• Preferred 2+ years in case management in acute care.

Responsibilities

• Coordinate patient care across continuum and remove barriers to timely care.
• Apply process improvement methods and evaluate care outcomes.
• Communicate with physicians, nursing and ancillary staff.
• Coordinate discharge planning and information with Admitting.
• Lead multi-disciplinary care conferences and morning meetings.
• Ensure accurate patient demographics and insurance data.
• Discuss medical plan of care with patients and families.
