Job summary

Hackensack Meridian Health is seeking an RN Case Manager for per diem coverage in Hackensack, NJ. You will coordinate patient care across the continuum, collaborating with physicians, nurses, and ancillary staff to ensure evidence-based practice and regulatory compliance.

Responsibilities include discharge planning, utilization management, and communication with patients and families about treatment and discharge plans, with a focus on reducing length of stay.

Qualifications

• Bachelor's degree in nursing (BSN) preferred; 1 year experience for non-BSN HackensackUMC employees.
• RN license in NJ is required.
• BLS certification required.

• Apply process improvement methodologies in evaluating outcomes of care.
• Coordinate communication with physicians, nursing, and ancillary staff.
• Evaluate care provided against the Length of Stay.
• Collaborate with Admitting to ensure accuracy of patient demographic and insurance information.
• Communicate with patients and families regarding medical and discharge plans.
• Assess and plan for discharge needs and arrangements.
• Complete utilization management for assigned patients.
• Monitor length of stay and clinical resource use on an ongoing basis.
• Manage all aspects of discharge planning for assigned patients.

Requirements

Licenses & Certifications

• Current NJ State Professional Registered Nurse License required.
• BLS AHA Basic Health Care Life Support HCP Certification required.
