Responsibilities

• Provide staffing support to Case Management Special Programs including Surgical Services Case Management, Outpatient High Risk Case Management, and Transfer and Access Case Management.
• Collaborate with clinical teams to coordinate post-acute care and discharge planning.
• Support capacity management and repatriation across Brown University Health system.
• Assist with data collection for CQI efforts to improve quality and cost-effectiveness of patient care.
• Maintain compliance with licensure requirements and hospital policies.
