Job summary

Avid Health at Home is hiring an in-office RN - Per Diem for client assessments at the St. Joseph branch, covering surrounding counties in Kentucky. Responsibilities include managing intake assessments, generating care plans, building client relationships, ensuring safety and HIPAA compliance, and documenting concerns promptly.

Travel is required; a valid driver’s license and a reliable vehicle are needed. Requires Kentucky RN licensure (compact license preferred), minimum two years of clinical

Qualifications

• RN licensed by the Kentucky Board of Nursing; compact license preferred.
• Two years of clinical nursing experience.
• Travel required for business purposes.
• Must maintain licensure and comply with applicable state/federal requirements.

Responsibilities

• Manage client intake assessments from all referral sources.
• Generate client plan of treatment and care plan assessments according to a physician’s order.
• Build relationships with clients to encourage compliance with care plans and client safety.
• Manage client/contract satisfaction with the highest level of customer service standards, communication, and documentation.
• Follow up and document all clinical concerns and incidents within the agency’s timeframe.
• Follow Agency Policies and Procedures and ensure HIPAA regulations are adhered to.
• All other duties as assigned.
