Job summary

Avid Health at Home Michigan LLC is seeking an RN - Per Diem for client assessments based at the St. Joseph branch, serving St. Joseph and surrounding counties. You will perform intake assessments, develop care plans, and coordinate with physicians and families to ensure compliant, quality care.

The role requires two years of clinical nursing experience, an active Michigan RN license, and travel with a valid driver's license and vehicle. Per diem hours vary to meet client needs and caseloads.

Qualifications

• RN license with Michigan BON in good standing.
• Minimum two years clinical nursing experience, including pediatric and adult care.
• Current driver's license and willingness to travel for business.

Responsibilities

• Manage client intake assessments from all referral sources.
• Generate client plan of treatment and care plan assessments per physician order.
• Build relationships with clients to encourage compliance with care plans and safety.
• Manage client/contract satisfaction with high levels of service, communication, and documentation.
• Follow up and document clinical concerns and incidents within agency timelines.
• Adhere to HIPAA regulations and agency policies.
• All other duties as assigned.
