Job summary

Avid Health at Home is seeking a Per Diem Registered Nurse to perform client assessments and develop care plans for patients in the St. Joseph area.

This in-home role requires an active Michigan RN license, reliable transportation, and at least two years of clinical nursing experience with pediatric and adult patients. Travel to surrounding counties is expected.

Qualifications

• RN licensed by Michigan Board of Nursing (BON).
• Valid driver's license with reliable vehicle.
• Minimum two years of clinical nursing experience.
• Experience with pediatric and adult care.
• One additional year in hospital care for training roles (preferred).

Responsibilities

• Manage client intake assessments from all referral sources.
• Generate client plan of treatment and care plan assessments according to physician’s order.
• Build relationships with clients to ensure compliance with care plans and client safety.
• Manage client/contract satisfaction with high service standards, communication, and documentation.
• Follow up and document clinical concerns and incidents within the agency’s timeframe.
• Follow HIPAA regulations.
• All other duties as assigned.
