Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) seeks experienced Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) across Kentucky, including Daviess County. This is a flexible, part-time role designed to supplement income with set hours you choose.

Responsibilities include conducting HRAs, documenting conditions, reviewing medications, and educating patients and families on prevention. Requires an active KY NP license and reliable transportation for home visits.

Qualifications

• Active Nurse Practitioner license in Kentucky.
• Experience with Health Risk Assessments (HRA) preferred.
• Strong clinical assessment and documentation skills.
• Ability to work independently and manage schedule efficiently.
• Reliable transportation for in-home visits.

Responsibilities

• Conduct comprehensive in-home Health Risk Assessments (HRAs).
• Identify and document acute and chronic medical conditions.
• Obtain vital signs and calculate BMI.
• Review medications and pharmacological therapy.
• Perform a physical examination.
• Evaluate patients’ physical, mental, and social health needs.
• Educate and counsel patients and their families on health conditions and preventative care.
