Job summary

SolutionHealth is hiring a Physical Therapist for the Bedford Outpatient Rehab clinic. This per diem position covers Tuesday and Thursday late afternoons and early evenings at the Bedford location, with hours aligned to location times.

The role focuses on evaluation, plan of care development, and collaboration with the care team to optimize patient outcomes. Eligible clinicians have NH licensure and CPR certification, and mentoring opportunities are available.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Licensed Physical Therapist by NH board.
• CPR certification required or within 90 days of hire.

Responsibilities

• Evaluate referred patients and determine treatment needs.
• Develop and adjust a written plan of care with measurable goals.
• Communicate progress with referring clinicians and family as appropriate.
• Support discharge planning and home exercise instruction.
• Provide supervision to therapy assistants and rehabilitation staff.
