Job summary

The Elliot Health System outpatient Rehabilitation Services in Bedford is seeking a Physical Therapist to evaluate and treat patients, develop treatment plans, and coordinate care with the health team. Responsibilities include patient assessment, goal setting, and home exercise instruction.

Schedule is per diem with Tuesday and Thursday late afternoons/early evenings, in a department offering orthopedic, neurological, pelvic floor, vestibular, geriatrics, pediatrics and oncology services.

Qualifications

• Graduate of an accredited Physical Therapy Program with Entry Level Degree required.
• Licensed as a Physical Therapist by the NH Physical Therapy Governing Board.
• CPR certification required or able to complete within first 90 days.

Responsibilities

• Evaluates patients and develops a written plan of care with short- and long-term goals.
• Implements and adjusts treatment programs based on patient response and clinician input.
• Communicates with referrers and care team regarding progress and plan changes.
• Provides discharge planning and home exercise instruction for patients and families.
• Supervises therapy assistants and other staff involved in care delivery.
• Participates in clinical development and education programs within the department.
