Job summary

OneWell Health Care is seeking a dedicated Per Diem Physical Therapist to provide home and community-based therapy in Pennsylvania, including West Chester and surrounding areas. This field-based role offers flexibility and the chance to impact individuals with IDD in their own environments.

You will evaluate mobility, design individualized treatment plans, and collaborate with families and caregivers to improve independence, safety, and quality of life while maintaining thorough clinical

Qualifications

• Doctorate or Master's degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice.
• Current CPR certification (preferred or required in accordance with organizational policy).
• Strong clinical, documentation, organizational, and communication skills.
• Proficiency with functional mobility/gait assessments, adaptive equipment, and EHR systems.

Responsibilities

• Evaluate mobility, strength, balance, gait, posture, and safety needs.
• Develop and update individualized treatment plans.
• Provide evidence-based physical therapy in home and community-based settings.
• Help individuals improve mobility, coordination, physical function, and independence.
• Recommend mobility aids, adaptive equipment, exercises, and fall-prevention strategies.
• Educate individuals, families, and caregivers on safe mobility techniques.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
