Job summary

OneWell Health Care is seeking a dedicated Per Diem Physical Therapist to provide personalized therapy in home and community-based settings in Pennsylvania. You will evaluate each individual’s physical needs, develop goal-based plans, and collaborate with caregivers to improve mobility, safety, and independence.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice (PA).
• CPR certification is preferred or required per policy.

Responsibilities

• Evaluate mobility, strength, balance, gait, posture, and safety needs in clients.
• Develop and update individualized, goal-based treatment plans.
• Provide evidence-based PT in home and community-based settings.
• Help individuals improve mobility, coordination, function and independence.
• Recommend mobility aids and therapeutic exercises; advise on fall prevention.
